Much of this Certificate of Death is hard to read Entry date; Jan. 19, 2001 ****************************************************************************************************** July 9, 1998 - I contacted Judy Morrell, at the Community Public Health Agency She is the Records Management Coordinator; I had called her as someone had told me they destroyed all the State Hospital Records; She sent me a letter with two Kardex card information that remained from my great grandmother's file at Traverse City State Hospital, and enclosed an applacation for her certificate of death; The card contained the following information; Name; Lung, Jane Case; 664 County; Benzie Date admitted; October 29, 1887 Accompanied by; Thomas Lung "husband" and C.C. Miller Dep. sheriff Taken to; Hall 11 Color;___ Sex; female Age; 38 Occupation; Labor No. of Children; ___ Age of youngest; ___ No. of years in Mich.;____ Education; Common Civil Condition; Married Religion; Dunkard "church of the Brothern" Nativity of Mother; ________ Nativity of Father; _______ Cause of insanity; Puerperal & anxiety Duration of insanity; one-week Form of insanity; _______ Insane Relatives; Father, intenperale Physical Defects and Diseased Organs; __________ Hight; 5' 5" Weight; 198 1/2 # Hair Color; Black Eye color; Gray Recent Injuries; ________ Patient record from The Traverse City Regional Psychiatric Hospital; April 22, 1901 Died In 1873, 1874 & 1883 she had attacks for which she was treated at the Indianapolis asylum 7 months 8 months & 9 months respectively each attack followed confinement. Cause of previous attack; puerperal & anxiety ineident to moving from Ind. to Mich. April 22, 1901 Patient died this morning at 6:15 of Turberculosis.
