Death date [5 March 2011]

At first glance, it looks like his death certificate gives the death date as May 18. However, it is actually "May 1st." The person filling out the certificate started using ordinal numbers on this section of the certificate, though for some reason used a cursive "s" instead of a printed "s." The date of burial is clearly listed as May 3, which would preclude a death date of May 18.
