Grandma must have been concerned about her birth certificate, the name on it and the date of birth. A letter written to her from the Department of Health in May of 1948 describes how she may amend her birth certificate. This letter was written in response to a form she filled out in April of 1948 requesting that not only her name be changed from Kava Lucy Edick (she hated the name of Lucy!!!) to Lucille Kava Edick but that her birth year be amended from 1914 to 1913. I think that's a first for any woman wanting to be known as older and not younger! Content of letter written to her: May 28, 1948 Mrs. Lucille Brown 139 Doblon Ct. Syracuse, New York Dear Mrs. Brown: We wish to acknowledge receipt of the attached correspondence relative to securing a record of your birth as prooof of your citizenship. We have located a certificate in the files of this office for Kava Lucy Edick born to Clarence Edick and Alice Hatch at Watertown, New York, which would appear to be the original record of your birth. However, the date of birth is entered on this record as February 15, 1914 instead of February 15, 1913 as claimed by you. We are enclosing a form by means of which your name may be corrected on this certificate. This form should be filled out on both sides and be signed in the presence of a nnotary public in accordance with the instructions attached. In addition to the form, properly executed, we will require documentary proof to support your correct given names. As the necessary proof we would ask you to furnish the following: 1. If you were baptized in childhood, a copy of your baptismal certificate showing your name, the date and place of your birth, the date of your baptism, your father's name and your mother's maiden name. 2. A certification from the family Bible. This certification should be made by a notary public, should be an exact copy of the entire Bible entry, and should contain a statement of the date or approximate date on which the entries were made in the Bible. 3. A certification from the 1915 New York State Census record showing your name, your age, your parents' names, and the names and ages of all other children in the family listed in that census record. If your parents were living in Jefferson County when this census was taken, the certification may be obtained by application to the Clerk of Jefferson County at Watertown, New York. 4. The name, date and place of birth of each child, both living and dead, born to your parents. We would have no authority to amend the year of birth on the original birth certificate unless we were in receipt of sufficient proof showing the correct year of your birth to be 1913. If the documents requested above show the year of your birth to be 1913, we may take into consideration the amendment of this item on the original record.
