He also was the assistance minister at the Church of Brethren-Long Beach, California, and at the First Church of the Brethren in Phoenix, Ariz. Dad loved to fly. He had his Instructors license, and was qualified to teach and fly dual engines aircraft. Ischemic Cardiomyopathy 2. Congestive Heart Failure Exacerbation. 3. Atherosclerotic Heart Disease Status Post Myocardial Infarction and Bypass Graft Surgery. 4. History of Ventricular Tachycardia, status Post PCD. 5. Disbetes Mellitus Type 2 6. Dyspepsia. 7. Pleural Effusion. 8. History of Abdominal Aoftic Aneurysm. Status Post Repair in 1998 History of Presenting Illness! Please look at my detailed history and physical for admission. This is a 70 year old gentleman who was admitted with worsening shortness of breath and chest discomfort. Due to his worsening breathing and inability to sleep, the patient was admitted to rule out myocardial infarction and also for treatment of congestive heart failure. Serial cardiac enzymes were done which were negative for myocardial infarction. A 2-D ec hocardiogram was also done which showed an ejection fracture of 40% and moderate_________ mitral regurgitation and motor tricuspid regurgitation. The patient was give diuretics and with that he symptomatically improved. A chest x-ray was done which showed right pleural effusion. Dr. Bondade was consulted who had seen the patient before when he did have plueral centesis in the past. Dr. Bondade's impression was that the patient could be managed conservatively and he will follow-up with the patient as an outpatient. Also, because of epigastric pain and history of gallstones, Dr. Saini evaluated the patient and his feeling was that he had dyspepsia. A HIDA scan was done which was negative and ultrasound of the abdomen was done which showed gallstones and normal common bile duct signs. The patient is being discharged home in stable condition. He will follow-up with Dr. Bondade in two to three weeks. Disseminated coccidioidomycosis is a fungal infection that spreads through the bloodstream and involves many organs. Causes Return to top Coccidioidomycosis is caused by breathing in spores of Coccidioides immitis, a fungus found in the soil in certain parts of the southwestern U.S., Mexico, and Central and South America. Most acute infections cause no symptoms and are only recognized by a positive coccidioidin skin test. Other times, the symptoms range from mild to severe. In the disseminated form of the disease, the infection may spread to the bones, lungs, liver, brain, skin, heart, and pericardium (sac around the heart).
