    DECLARATION FOR INVALID PENSION
    To be executed before a Court of Record or some officer thereof having custody of its seal.

    On this 13 day of April, A.D. 1891, personally appeared before me, a circuit court clerk of the court, a court of record within and for the County and State aforesaid, Jonas Warner, aged 50 years, a resident of the County of Montgomery, State of Kentucky, who, being duly sworn according to law, declares that he is the identical Jonas Warner, who was enrolled on the 9th day of August, 1863 in Company B 10th Regiment Kentucky Cavalry Volunteers in the war of the rebellion, and served at least ninety days, and was honorably discharged at Maysville Kentucky, on the 17th day of September, 1863. That he is parshaly unable to earn a support by reason of rhumatism and _______ and a injury to boath feet by a fall and a injury of the ankels. That said disabilities are not due to his vicious habits, and are to the best of his knowledge and belief permanent. That he has not applied for a pension. That he makes this declaration for the purpose of being placed on the pension roll of the United States, under the provisions of the Act of June 27, 1890.

    He hereby appoints, with full power of substitution and revocation, Wm. W. Dudley, of Washington, D. C., his true and lawful attorney to prosecute his claim. That his Post-Office address is Flat Creek, County of Bath, State of Kentucky.

    court clerk in and for the aforesaid County, duly authorized to administer oaths Jonas Warner aged 50 years, a resident of_____________, in the county of Bath, and state of Kentucky whose post office box is Flat Creek, Bath County, Kentucky well known to be reputable and entitled to credit, and who, being duly sworn, declared in relation to aforesaid case, as follows:

    I enlisted in Company B, 10th Regiment Ky Cavalry Volunteers on or about the 10th day of August 1862 and served faithfully until the 17th day of September 1863 and was honorable discharged at Mayesville, Kentucky. The above named service is and the service that I ever rendered in my life in any army or war.

    Personally appeared before me a circuit court clerk in and for the aforesaid County and State, Jonas Warner aged 50 years, citizen of the town of Flat Creek, County of Bath, State of Kentucky well known to me to be reputable and entitled to credit, and who, being duly sworn, declared in relation to aforesaid case, as follows:

    On or about September 1888 I feel from a home where I was building a barn
    and I fell about 20 feet. I lit on my feet and the injury to my feet caused by the fall has injured my feet and ankles so bad that I am unable to walk a good part of my time and I am not abil to perform heavy manuel labor and not much lite labor.

    On this 9 day of April, A.D. 1892, personally appeared before me a circuit court clerk in and for the aforesaid County, duly authorized to administer oaths Jonas Warner aged 50 years, a resident of_____________, in the county of Bath, and state of Kentucky whose post office box is Flat Creek, Bath County, Kentucky well known to be reputable and entitled to credit, and who, being duly sworn, declared in relation to aforesaid case, as follows: My name is Jonas R. Warner. In signing my name I never sign my name as Jonas R. Warner as it is a shorter way to rite. I Jonas Warner for a short named I make this affidavit to fill a deficiency. I by my declaration ____ executed and filed for a pension act of June 27th, 1890 as it was an over site by me to sine my name Jonas Warner when it should have bin Jonas R. Warner my name in the discharge and in the pension record and War record is rite Jonas R. Warner. I am the identical Jonas R. Warner who rendered the service in Company B 10th Regiment Kentucky Cavalry Volunteers from the 10th day of August 1862 until the 17th day of September 1863 and was honorably discharged at Maysville Kentucky. The above named service is the only service ever rendered by me in any army or war.

    On this 15 day of February, A. D. 1911, personally appeared before me, notary public within for the county and State aforesaid, Jonas R. Warner, who, being duly sworn according to law, declares that he is 70 years of age, and a resident of Carlisle, county of Nicholas, State of Kentucky: and that he is the identical person who was enrolled at Covington, Kentucky under the name of Jonas R. Warner, on the 10th day of August, 1862, as a Private, in Company B, 10th Regiment, Kentucky Volunteer Cavalry in the service of the United States, in the Civil War, and was honorably discharged at Maysville, Kentucky, on the 17 day of September, 1863. That he was not employed in the military or naval service of the United States otherwise than as stated above. That his personal description at enlistment was as follows: Height, 5 feet 11 inches; complexion, dark: color of eyes, black; color of hair, black; that his occupation was farmer;
    that he was born February 15, 1841 in Bath Co., Kentucky

    That his several places of residence since leaving the service has been as follows: Bath County, Kentucky until September 1865, Douglas County, Illinois until December 1866. In Bath and Montgomery counties, Kentucky until 1899. Since then in Nicholas County.

    That he is a pensioner. That he has heretofore applied for pension Certificate 818591.

    That he makes this declaration for the purpose of being placed on the pension roll of the United States under the provisions of the act of February 6, 1907.

    My pension title is as follows: Jonas R. Warner, Private Company B, 10th Regiment Kentucky Cavalry, Civil War, and my certificate is number 818,591.

    My pension certificate bears date October 1, 1912, to begin May 21, 1912, at the rate of Twenty Dollars per month.

    On this 5th day of May, A. D. 1922, personally appeared before me, a Notary Public, within and for the county and State aforesaid, Jonas R. Warner who, being duly sworn according to law, declares that he is 81 years of age, and a resident of Carlisle R. F. D. 2 county of Nicholas, State of Kentucky; and that he is the identical person who was enrolled at Mustered in at Covington, Ky under the name of Jonas R. Warner on the 10th day of August, 1862, as a Private in Co. B, 10th Regiment Kentucky Volunteer cavalry in the service of the United States, in the Civil War, and was honorably discharged at Maysville, Kentucky, on the 17th day of September, 1863 that he also served no other service.

    That his personal description at enlistment was as follows: Height, five feet 11-3/4 inches; complexion, dark; color of eyes, brown; color of hair, black; that his occupation was farmer; that he was born February 15, 1841, at near Owingsville, bath County, Kentucky.

    That he requires the regular personal aid and attendance of another person on account of the following disabilities:
    Disabled right shoulder by reason of which he is unable to dress and undress himself: disabled right knee due to an accident by which he is hardly able to walk at all: disabled back, unable to bend it: totally deaf in right ear, left ear hearing very defective. Left shoulder also very much disabled: right testicle enlarged 10 times usual size.

    That since leaving the service he has resided at Bath, Montgomery and Nicholas County, Kentucky. Has lived in Nicholas County, Kentucky for the past 22 years and his occupation has been farming.

    That he is a pensioner under Certificate No. 818, 591. That he makes this declaration for the purpose of being placed on the pension roll of the United States under the provisions of the Act of May 1, 1920.

    In the claim for increase pension under the Act of May 1, 1920, of Jonas R. Warner, late Company B, 10th Kentucky Cavalry, and who is pensioned under Certificate No. I. C. 818,591, on this 13th day of June, 1922, personally appeared before me, a Notary Public in and for the State and County aforesaid, duly authorized to administer oaths, Dr. O. S. Kash, aged 66 years, and whose place of residence and post office address is Carlisle, Kentucky, and who being duly sworn, says in relation to above mentioned claim as follows:

    "That his name, age, place of residence and post office address is correctly stated above, and that he has been actively engaged in the practice of medicine since 1882, having graduated at the Kentucky School of Medicine at Louisville, Kentucky, in 1882.

    That he has been the claimant's family physician for the past 22 years and that he is afflicted with the following disabilities, and was so afflicted on the date of filing his claim on May 10th, 1922.

    Total deafness in right ear. Chronic appendicitis with adhesions. Hydrocele right, severe. Right shoulder dislocated. Right knee and lower leg shriveled due to injury; this latter injury result of quarry explosion, blast, premature. Requires assistance in dressing and undressing himself. Indications of cancerous affection of bowel due to trouble with appendix. It is unsafe for claimant to go out on the road or away from home without an attendant due to his deafness and combined disabilities.

    Affiant's means of knowledge to facts testified to herein are that he is his family physician, see him and treat him frequently, has examined him professionally and know the facts herein testified to.

    And he further says that he has no interest in this claim nor is he concerned in its prosecution.

    Is there any evidence of an old dislocation of claimant's shoulder? Are these the only joints impaired?

    Is there any reason, besides the impairment of claimant's right shoulder joint, why he needs help in dressing?

    If claimant has free use of his left arm, kindly explain why he can not put on and off his clothes.

    The claimant herein mentioned has the following conditions:

    1st, He is unable to lift his hand, right side, to his head: unable to put on his coat without assistance, with right arm: the right arm is carried almost motionless at his side: he might possibly be able to put on his coat without assistance, but it would be very difficult.

    2nd, No evidence of an old dislocation: claimant says he was struck over the shoulder with a gun, disabling him at that time for using his arm at all, and he suffered more or less since that time.

    3rd, There is no particular reason as we could discover of any disability
    requiring the free use of his left arm.

    4th, He claims that he cannot use his right arm in pulling on his coat without assistance.
