I have seen the image of his d/c at WV Vitals online but have not seen a copy of the original d/c. The image says he died on Indian Fork but does not specify which county. The index at WV Vitals says he died in Gilmer County - but that info is not on the image. There are Indian Fork communities in several WV counties. I picked Lewis just because that is where his wife died. That image also says he was single. He was 80 years old and had been a widower for 8 years. Maybe the informant just said he wasn't married??
