'Hendrick died at the home of his son John in May of 1804. His will was probated in Shelby County Court on Oct. 14, 1805. He was 86 years old.'

Will of Henry Banta of the County of Henry, State of Kentucky, dated 13 Dec 1799, proved in a court held for Shelby County, 14 Oct 1805.
Note that the town where he lived crossed the border between Shelby and Henry counties.

'HENRY BANTA died in 1805, aged 87 years, and was buried on George Bergen's farm in Shelby Co., being the first burial in that graveyard.'

This memorial claims he was born 9 Dec 1718 at Hackensack, Bergen, New Jersey, which is consistent with information provided elsewhere, but no source is provided for the exact date. The Banta Genealogy by T. M. Banta is mentioned, but it gives neither a birth nor a baptism date (see #161 on pages 28 and 49). His baptism is not recorded in Hackensack Dutch Reformed records between 1716 and 1721.
The memorial also states that he died 14 Oct 1805, but that was the date his will was proved, so the information should be received with caution.
