In the year onethousand eight hundred and forty, on the second day of the month December was shown for the undersigned, Johannis Agatinus van Meel, council of the civil administration of Melissant, province Zuid Holland, Leendert Otte, 35 years old, laborer, living at Melissant, who declares that at the second day of the month December 1840 at five o'clock in the afternoon was born at house number 14 of this municipality, a male child from his wife Johanna Jansen, laborer and also living at Melissant.

The child was given the name: Dirk. We declares that thedeed is made in presence of Marinus Kooman, 46, construction worker, and Gerbrand Zaaijer, 37, construction worker, both living at Melissant and this deed wasafter being reading out loud by us signed, the witness explained that the father was not able to do this [sign] because he never learned to write or to sign.
