D. C. Carr's birth was recorded as Milton B. Carr but the family decided to call him DeWitt Clinton (after the NY governor) after they registered his birth. His father's (Daniel Carr) civil war pension records contained the birth certificate and several affadavits explaining the discrepency. He was then called Bert most of his adult life.
