- ↑ According to his daughter Terry, “He always went by Carmen Joseph as an adult, one of his sisters said that he was actually named Carmen Vincent. He never knew that. The story is that when he enlisted, they insisted on having a middle name for their records, so he told them Joseph since he didn’t think he had a middle name.”
- ↑ He voluntarily enlisted in the Army Air Force but was unable to be deployed due to his poor eyesight.
- ↑ He unsuccessfully tried to start a practice in Middletown, Ohio before moving to Hamilton.
