Based on Sophia's death record and Boleszew's age in census records, it appears that their birth dates were reversed in this application. Possibly the birth places were also reversed.
He claimed to have immigrated 12 Jul 1891 and lived in Morris, Illinois from 1899 to 1909. However, on census records, he says he immigrated in 1901, and his wife had a baby in Europe in 1900 or 1901 before joining him in the United States. With the other errors on the form, it appears that there may have been some miscommunication, possibly due to a language barrier.
