Alf graduated from the University of Wisconsin in 1898 and graduated from the University of Illinois Medical School in Chicago in 1901.

He had persuaded his brother, Lucius Boardman Donkle, to go to medical school also and become a physician. Tragically, he died of appendicitis less than two years after graduating ending their dream of a joint practice.
