General Affidavit

STATE OF Indiana, COUNTY OF Vigo SS: In claim No. 670084 of Heirs of Jacob Varner Co. I/B/I of the 27/13/16 Regt. of Ind. Vols./" Cav./" Inf., Personally appeared before the undersigned duly authorized to administer oaths within and for said County, Alice Varner aged 30 years, whose P.O. is Terre Haute, County of Vigo, State of Ind, who being duly sworn states in relation to said claim as follows to wit:
That The Attending Physician is dead (whose name was Piatt) in Shelbyville Indiana [inserted:is dead]. There was no public record of either of our birth's neither church record and don't know where either of the female attendants are living.

And affiant further states that he has no interest in this claim.[1]
