MEDAL OF HONOR CERTIFICATE
ISSUED UNDER THE PROVISIONS OF THE ACT OF CONGRESS APPROVED APRIL 27, 1916.

To whom it may concern:
This is to Certify, That George L. Banks
was enrolled on the sixth day of
June, 1861, to serve three years, and was
discharged on the twenty-fifth day of June, 1864,
by reason of muster out of company
while holding the grade of First Sergeant, in Company C
Fifteenth Regiment of Indiana Infantry Volunteers,
that a medal of honor was awarded to him on the twenty-first
day of September, 1897, for most distinguished gallantry
in action at Missionary Ridge, Tenn., November 25, 1863, where he as
color bearer, led his regt. in the assault, and, though wounded, carried the flag
forward to the enemy's works, where he was again wounded, this flag being
the first planted on the parapet.
that his name was entered and recorded on the Army and Navy medal of Honor Roll
on the eleventh day of May, 1916, as
authorized under the provisions of the Act of Congress approved April 27, 1916,
and that he is entitled to receive the special pension granted by that Act.
Given at the War Department, Washington, D. C., this twenty-fourth day of
May, 1916
