    p. 358: In the family of "David de Malpas, of Hampton and Bickerton" and "Katherine, daughter of Hugh de Bickerton" is listed "Matilda, wife of Philip de Egerton, dispensation for marriage dated Sept. 1, 1403."
    On page 357, the author suggests that the dispensation was required because Matilda and Philip were related [the implication being that they could have been second cousins].
