His birth year is based on the assumption he was at least 18 in March 1704/05 when he and his brothers divided land given them by their father (the explanation is given for his brother David and applied to Hezekiah as well).
The previous page describes his parents as "of Plymouth" in 1679, and they appear to have continued to have lived there.
This record gives an unsourced death date of 23 April 1761 in Kingston, Plymouth, Massachusetts Bay. There don't appear to be any vital records to support that.
