_DCAUSE: Cardiac Arrest

A H Hopkins Sr developed depression and confusion around 08-01-2005. He was interviewed by Claire Killbride, head of nursing at the Villages of Kensington Ave, an assisted living facility in Meriden. He subsequently was admitted to the Villages to their dementia unit. There he had several falls leading to a stay in the emergency room at Mid-State Medical Center, Lewis Ave., Meriden, CT.

After returning to the Villages he was moved to an apartment with a live-in aide, Livingston Brown. After one week, he was brought to Dr. Burke of Physician's Walk-in who had him transported to Mid-State as his EKG showed a cardiac event had occurred. He was subsequently admitted for observation for 3 days.

He was then admitted to Miller Memorial Community Center, a skilled nursing facility on Broad Street, Meriden. He failed rapidly and within two weeks was transferred to Hospice oversight on September 13, 2005. He passed away the following day at 5:40 am on September 14, 2005.
